About Your Visit
What should you bring to your appointment at Cleveland Nasal-Sinus & Sleep Center?
Please prepare for this your visit by bringing the following information with you:
- Health Insurance Card/Information
- A friend or family member to take notes/help with questions
- A list of all medications you are taking
- A list of your questions /concerns
- A copy of your XRAYS / CT scans (not just the reports - get the actual films), and audiograms
- Copies of records from doctors related to your sinuses/nose/allergies/lungs and sleep issues.
COVID-19 Protocols
COVID-19 is contagious and still present in our community.
Cleveland Nasal Sinus & Sleep Center is dedicated to your health, safety and good care and Dr. Howard Levine takes all precautions for your protection during your visit.
Here is what you can expect:
- The number of patients allowed in the office will be kept to a minimum.
- Do not come for your visit if you have symptoms related to COVID-19.
- Do not come to the office if you have been recently exposed to anyone with COVID-19.
- You should plan to come alone unless you absolutely need someone to assist you.
- You will be required to wear a mask at ALL times covering both your nose and mouth.
- Dr. Levine will ask you to remove your mask when it is appropriate during your examination.
- Hand sanitizing will be available for you.
